How long did it take you guys to get DEA? As a FMG, do you need to submit
any additional documents? Or do you need to submit FCVS profile to them just
for DEA? Thanks. |

Actually, I remember it now. I called them several times, and they were very nice.
Sorry, I am talking about the DEA modification of registration. For residency, your program will handle the issue.
A practitioner who moves to a new location must request a modification of registration. It is the practitioner’s responsibility to notify the DEA about a change of address before the effective date of the move. The notice of change of address should be made in writing to the local DEA office. An Address Change Request is available on the Internet at www.DEAdiversion.usdoj.gov. The form must be manually signed and faxed or mailed to the local DEA office. The request must include a copy of the practitioner’s current state medical/professional license for the new address along with a copy of the practitioner’s corrected state controlled substance registration if applicable.

used an institutional number. Now I need an individual DEA, just realized it
may take too long, this will mess up my work plan.

Well, then I believe I started my new job w/o a DEA. I called them several
times.

发帖数: 28 | 8 Thanks for the information. I checked the website, it seems they only
require you to fill out the form and pay the fee. I am still waiting for my
state license to start the application. Did you need to submit any more
documents as an FMG? If so, I want to prepare in advance to try to save some
time. Thank you.
