标 题: 求助：脑血栓！！！

病人是朋友的父亲，人在国内，抢救了几天还处于基本昏迷状态:
男性，68岁，大面积的脑血管梗塞（脑血栓），6月21日凌晨1点多发病，当时左半侧无
法移动，后经120急救送至医院，3点钟左右开始输液，接近5点钟开始静脉注射70mg阿
替普酶alteplase（强溶栓 剂），但是没有溶开。后脑内出现大面积水肿，至23日中午
基本昏迷，（前两天神志还清醒，只是晕晕乎乎，还能吃些东西，能认人，能说话）昏
迷时右侧瞳孔有放大迹象，立即进行了开颅去骨瓣手术（右侧脑骨，去掉一大块，这样
能让水肿有地方扩大，减少颅内压力），术后瞳孔有回复迹象，但因为水肿仍处在高峰
期，颅内压力依然很大，而且持续上升，所以，瞳孔又进一步扩散。
至今高峰期已过，但水肿削减要有个过程，所以，脑干始终处于被挤压状态, 最担心的
是对脑干破坏时间越长越难恢复。现在病人未苏醒，但瞳孔趋向正常，肺部开始有感染
、发烧，心脏房颤持续有发生，这些医生都在治疗。
现在最担心的就是：即使包括心脏、肺这些并发症都能闯过去，大脑脑干有没有办法恢
复？如果恢复不了，就可能成植物人...
请问：
是否从现在开始能有些什么更好的措施和药物，尽可能的使脑干受损减少，恢复加速？
（目前这边始终使用的是大剂量的甘露醇和人血白蛋白）
另外，美国这边有没有比国内先进的治疗手段/方式？

少进一步损伤。 预后只有时间能够告诉你了。 Bless

FA13里删得只剩一句话了，可怜我一直只看FA13。看来要追高分，一定得看好几个FA版

chinese physicians. Instead, I just try to clarify some points on the
treatment of stroke in the US for our Externs.
1. TPA probably is not going to work for such a large stroke, what is the
alternative treatment?
2. What do you think about the craniotomy?

1. At this point there is no alternative that is proven better than tPA.

2. You may say the pt is a little too old for hemicraniectomy, but it is not
excessive.
"Patients with malignant MCA territory infarction should be enrolled in
randomized controlled trials of decompressive hemicraniectomy if this option
is available. In the absence of clinical trial availability, and given the
dire prognosis associated with medical treatment, we suggest decompressive
hemicraniectomy for patients age 50 or younger with an indisputable
diagnosis of malignant middle cerebral artery territory infarction (with >50
percent infarction of MCA territory by head CT) who are at high risk of
developing malignant edema and who desire aggressive therapy. This surgery
may also be offered on a case-by-case basis for otherwise healthy patients
older than 50 and beyond 48 hours after stroke onset." - uptodate

will learn what alternative treatment is and what is excessive in your later
clinical practice.

大，我已经删了
tPA就是alteplase. 做MRA(是一种MRI)并不是必须的标准程序.

但基本成植物人或预后很差。来美进修感觉他们很不积极，讨论起来
他们讲没有生活的quality，所以不积极。当时我非常shock,因为我们
国内不会因为life quality不好而放弃救命。 |

发帖数: 297 | 16 理念不一样，西方因为宗教的关系，不认为死亡意味着一切的结束，所以注重生活的质
量。而中国人认为死了就是到此为止，其他的什么也别谈了，只要“活着”就留的青山
在。

发帖数: 50 | 17 到这个点上，在美国也没有其它办法。不要输液太多造成更多水肿，只是觉得中国溶栓
速度太幔。只能听天由命，穑幋过右侧大脑中风，预后比左侧相对好一些。
