You will each need to provide the following documents:
1. I-693 Medical Examination Form:
a. This medical examination must be done with a USCIS approved physician (

b. When you schedule the appointment, make sure the physician has the I-693
forms
c. Make sure the envelopes remain sealed (DO NOT OPEN)
2. Copy of all immigration documents covering your immigration history in
the U.S., which may include:
a.Passport (full copy from cover to cover, including blank pages)
b.Previous passports, if applicable (full copy from cover to cover,
including blank pages)
c. I-94 Card(s) (if applicable)
d. I-20, DS-2019, or IAP-66 (if applicable)
e. Any employment authorization documents issued (EAD cards, H-1B approvals,
etc.)
f. Copy of any I-797 Approval Notices for previously filed applications (if
applicable)
3. Copy of birth certificate (with translation, if not in English).
If the document has been translated, please provide a signed copy of the
certification of the translator (attached). Please note that you cannot sign
this document yourself. The items highlighted in yellow should be changed
by the translator. If you obtain a translation through a professional
translation company, they will generally provide some statement of
proficiency, which is also acceptable.
4. Copy of divorce or death certificate (with translation) for prior spouses
(if applicable)
5. Six (6) passport style photos: two (2) pictures are to be used for the
application for permanent resident status, two (2) are to be used for
employment authorization, and two (2) pictures are to be used for advance

Emailed copies of the documents listed above are perfectly acceptable, but
you are welcome to mail the document copies, too. When the forms are
finalized, you can send the signed form, medical results, and pictures to me
in one envelope. You’re welcome to send things separately, too. However,
to produce the forms for the petitions, I will need the questionnaires and
the immigration document copies. |
